Heat Inc. provides Support Coordination and Home/Community-Based services for individuals diagnosed with Developmental Disability Disorders.

Support Coordination

Supports Coordination is the process of working with participants, families, and community contacts to identify and implement support strategies, while advocating to get the participants needs met. Through this process, our coordinators complete a Needs Assessment, Individual Support Plan, Locate Services, Coordinate Services, Monitor Services and Advocacy.

Supports Coordinators

Support Coordinators work with families to ensure consumers receive the necessary supports at home, at work, and in the community. They help to identify the families’ needs and connect families to the most appropriate and available services through the development of an Individualized Service Plan. 

Support Coordinators ensure that families receive the appropriate services and monitor services to make sure they are meeting their individual needs.  Support Coordinators report services to Division of Developmental Disabilities to ensure quality care.

Community Care Program

The Community Care Program is a Medicaid home and community-based services (HCBS) waiver program. It provides services for eligible individuals who live in a provider-managed setting, such as a group home or supervised apartment, or who live in their family home or their own apartment or home, to assist them to live as independently as possible. 

Home and community-based services are intended to supplement rather than replace services received from other state programs, such as NJ FamilyCare or Division of Vocational Rehabilitation Services, or from natural supports, including families, neighbors or community organizations.

Depending on an individual’s assessed level of support need, Heat Holistic Lifestyle provides the following services in the Community Care program:

  • Behavioral Supports
  • Career Planning
  • Community Inclusion Services
  • Individual Supports
  • Occupational Therapy
  • Physical Therapy
  • Respite
  • Speech, Language and Hearing Therapy

Eligibility

To be eligible for the Community Care Program, an individual must:

  • be reached on the CCP Waiting List or be determined by DDD to be at imminent risk of homelessness
  • meet the functional criteria for eligibility for DDD services
  • meet the additional Level of Care criteria for Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/ID)
  • meet specific Medicaid requirements regarding income and resources (DDD works with individuals to help establish whether or not they meet these Medicaid requirements)
