Let Us Help You Find the Perfect Service That Fits Your Lifestyle
Heat Holistic Lifestyle Program offers a range of Direct Support services, including Community-Based Supports, Individual Supports, Respite, and Behavioral Support, all designed to promote community involvement, inclusion, and independence for individuals with intellectual and developmental disabilities. Our program provides a comprehensive continuum of integrated human services to address personal, economic, and social challenges. By alleviating stressors and constraints, we empower individuals to overcome obstacles, build resilience, and achieve personal growth, enabling them to lead more fulfilling and independent lives.
Heat Holistic Lifestyle Program provides Intensive In-Community (IIC) services, a focused, analytical, and goal-oriented clinical intervention designed to address the emotional and behavioral challenges of youth with moderate to high needs. Delivered by licensed behavioral health clinicians, IIC services are provided in the youth’s home or a suitable community setting. These services aim to stabilize the youth and family, deter the need for more intensive interventions, and foster collaboration with the Child and Family Team (CFT) or the youth’s identified support system.
As a short-term, solution-focused intervention, IIC services target specific presenting behaviors to help the youth and their family or caregivers gain insight, improve functioning, and build resilience both at home and within their community. These services are part of a comprehensive, individualized plan of care, driven by the youth and family, and designed to address their assessed needs. To ensure the best outcomes, interventions are delivered at times and locations that are most convenient and effective for the family.
IIC services are tailored to meet the unique needs of each youth, incorporating their cultural values, norms, and strengths. The primary goal is to reduce symptoms, promote individual skill development, and restore or maintain the youth’s ability to function effectively. IIC clinicians employ a wide range of evidence-based interventions, including psychoeducation, negotiation and conflict resolution training, coping skill development, emotional regulation, stress management, and problem-solving. Additionally, clinicians focus on teaching healthy limit-setting, symptom management, self-care, and skills that enhance self-fulfillment and overall well-being.
These intensive services are typically short-term, lasting no more than three months, with therapeutic sessions provided one to two hours per week. By addressing immediate challenges and equipping youth and their families with the tools to thrive, IIC services serve as a critical component in achieving long-term stability and success.
